Spinal Problems & Referred Leg Pains

Session 1: Vertebral compression fractures and spondylolisthesis

Mr James Langdon will be speaking on: Back pain is one of the most common musculoskeletal causes for patients to consultant a therapist or doctor. Some patients with chronic back pain can develop acute problems. Two common causes of acute-on-chronic back pain are vertebral compression fractures and spondylolisthesis. This talk will give the therapist further knowledge about these conditions and the options for treatment.

Session 2: Spinal pathology and referred leg pain – the differential diagnosis

Leg pain is often due to spinal pathology (disc prolapses, spinal stenosis, facet joint arthritis, and spondylolisthesis), but it can also be the presenting complaint for a number of non-spinal conditions (peripheral vascular disease, hip and knee OA, trochanteric bursitis, and piriformis syndrome). This talk will outline the common causes of referred leg pain, how they present as well as providing useful examination tips helping you to make the correct diagnosis for your patients.

Confused? ! Orthopaedic Surgeon v Neurosurgeon v Pain Management Consultant

During an extended evening session attendees will have the opportunity to learn from and engage with two local Consultants specialising in the management of spinal pain. 

Mr Jeremy Reynolds, Spinal Surgeon, explains why he believes spinal surgery has sometimes failed to deliver a satisfactory reputation to its users, whether that be those referring or the patient’s themselves. Many misconceptions prevail often due to less than ideal application of complex interventions and expensive technologies and often a fundamental misunderstanding of pain generators.  He will explain how spinal surgery has traditionally come under the auspices of two separate camps, neurosurgery and orthopaedics, and how this has further served to muddy the waters. Dr Yaser Mehrez, Consultant in Pain Medicine & Anaesthesia, will explain the various mechanisms involved in development of chronic pain and ways of assessing the extent of chronic pain problems in general. The presentation will share with attendees the pain clinic approach in assessment, investigations of patients with chronic spinal pain and different interventions that can be delivered by the pain clinic.  As always there will be ample time for formal and informal discussion with our speakers throughout the evening.

Rotator Cuff Treatment: New Advances and Surgical Strategies

Professor Ofer Levy & Mr Giuseppe Sforza  based at the Reading Shoulder Unit are speaking on:- The treatment of Rotator Cuff Disease has changed significantly in the last few years.  This has come about as we now have more specific information available in relation to the locality in which the lesion starts, how much and how quickly it progresses as well as the natural history of this disease.

This event will provide the audience with a better understanding  of the pathogenesis, diagnosis and evidence based treatment algorithm.

Whiplash Injury!

Dr Jenner  will present an interactive lecture focussing on whiplash and whiplash associated disorders. These conditions are responsible for significant amounts of morbidity following RTAs and are a common cause of medico-legal claims. Dr Jenner will look at the definition and causes of whiplash injury, he will review the clinical findings in these patients and illustrate them with case histories.

He will review the investigation findings commonly seen and the multi-disciplinary approach to treatment that he uses. This will include a discussion of the role of analgesic medication in combination with physical rehabilitation therapy, as well as the overlap with clinical pain psychology.

Dr Jenner will also discuss contemporary treatments used in the management of refractory whiplash cases including image guided steroid injections, radiofrequency denervation techniques and the role of Botulinum Toxin A therapy. There will be information on the medicolegal aspects of whiplash injury including the rise in use of covert surveillance by defendant’s solicitors in such cases.

There will be plenty of opportunity for questions and Dr Jenner would welcome discussing any difficult cases attendees would like to discuss.

The Event 


Part 1 
 
Achilles tendinopathy is a very common injury in athletes of all sorts but especially runners, both elite and recreational. Treatment has changed dramatically over the last 10 years. Conservative treatment has improved so much that surgery is very rarely indicated these days. The anatomy and pathology will be discussed.
 
Part 2 
 
Treatment modalities will be discussed  and answers to the following questions :
 
What are the alternatives and when should they be used
What options are open to the General Practitioner and physiotherapist?
When should injections be used and what should be injected?
What other diagnoses should be considered and what investigations are best.
Focus will be placed on high volume saline injections which carry a better than 80% cure rate but consideration will be given to blood and PRP along with minimally invasive surgery.

The Event


This session has been designed to provide an interactive, open forum where Dr Christopher Jenner, Consultant in Pain Medicine will initiate discussion in response to case studies and questions in relation to Contemporary Pain Management. You will be given an opportunity to submit case studies and questions prior to the event. We will provide you with an overview of cases and questions prior to the session to enable you to contribute to the discussion if you so wish. Whilst this session is interactive you are not required to actively participate if you do not wish to. The session will begin with a brief overview of relevant aspects of pain medicine, including basic physiology and pharmacology, and a description of a number of common pain complaints including musculoskeletal pain (lower back pain, whiplash, thoracic pain) and neuropathic pain (including complex regional pain syndrome and neuropathies). Further to the Introduction Dr Jenner will respond to case studies provided by the assembled audience – where required he will explain the signs & symptoms, the underlying physiology and potential treatment required. Where presented with a question he will provide his view or response. At this point the Chairperson will open the issue to the multi-disciplinary audience with a view to initiating constructive discussion!
